I, the undersigned parent or legal guardian of the above-named child, give permission for my child to participate in all activities associated with Vacation Bible School (VBS) at Pentecostals of Columbia.
I understand that participation in church activities may involve certain risks, including but not limited to physical activity, games, crafts, and outdoor play. I voluntarily assume all risks associated with my child’s participation.
I hereby release, waive, discharge, and hold harmless Pentecostals of Columbia, its pastors, staff, volunteers, and representatives from any and all liability, claims, demands, or causes of action that may arise from injury, illness, or damage sustained by my child while participating in this event.
